Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about January 2, 2024, and at times prior, the employer did not ensure procedures were developed and utilized to control hazardous energy when conducting maintenance operations on machines and equipment such as but not limited to the 230/460V 3-phase MAC MD40 Airlock, s/n 1100310551-020-1. An employee suffered multiple finger amputations on the right hand when reaching into the airlock system to attempt to remove a jam while a supervisor was operating the controls. The airlock controls were being operated on a single point control system and were being placed in an ON and OFF position to break the debris collected as per company procedure.
