29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). The employer is failing to protect employee(s) from cave-in hazards. This was most recently documented on December 26, 2023, at 12112 S. 109th St. Papillion, Nebraska. The employer failed to ensure that employee(s) working in a trench were not exposed to cave-in hazards at depths greater than 11 feet.
