OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) On or about December 20, 2023, employees were exposed to struck-by hazards in that industrial storage racks were visibly damaged with bent and twisted beams and columns, and damaged or missing anchor bolts in locations included, but are not limited to location T-19, T-20, and T-21. Furniture items such as but not limited to headboards, bed frames, mattresses, and cabinets were stored on multiple levels of the racking system. (b) On or about December 20, 2023, employees were exposed to struck-by hazards where industrial storage racks were missing weight limits throughout the warehouse. Furniture items such as but not limited to headboards, bed frames, mattresses, and cabinets were stored on multiple levels of the racking system.
