29 CFR 1926.20(b)(2): The employer did not maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person with a frequency that deterred employees from violating the employer's work rules: a. On a steep roof with a 5/12 pitch, of a new construction residence, located at, or near, 3027 Fiera Vista Drive in Jacksonville Florida: On December 8, 2023, the employer exposed employees to a fall hazard of 23-foot, in that the employees personal fall arrest systems were not properly adjusted to prevent them from contact with the lower level, while performing roofing work.
