29 CFR 1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a. On or about December 4, 2023, located at the jobsite 4547 West Northbank Road, Buckeye Lake, Ohio, the employer did not ensure the powered industrial truck operator was competent to operate the vehicle. The operator of a Genie Model GTH-644 rough terrain forklift was not trained and evaluated exposing employees to struck by and fall hazards.
