29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Barium Carbonate Settler: On October 24, 2024, and at times prior, the employer exposed employees to thermal burn hazards, in that the employer's energy control procedures for overflow pumps and connected pipelines did not clearly and specifically outline the steps for isolating or controlling hazardous energy, the specific steps for placement, removal, transfer of the locks, and the specific requirements for testing a machine or equipment to verify the effectiveness of the energy control measures as authorized employees performed maintenance and servicing work on the overflow pumps.
