29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 38 Jackson Rd, Devens, MA: Instance a - On and before November 11, 2023, employees did not utilize all of the multiple documents that together contained the required elements of machine specific energy control procedures (ECP). On four occasions on November 11, 2023, an employee who was performing metrology work did not use the 287 System/Equipment Release Form used for Affected Employee Notifications. On October 24, 2023, a Maintenance Technician did not use a Safe Work Permit required to be used with the Draft Energy Control Procedure when preparing the Water For Injection 831 machine for Metrology Technicians to perform servicing activities. Instance b - On and before October 24, 2023, employees did not utilize ECPs and/or place locks onto isolating devices when they were exposed to potentially hazardous energy. On October 24, 2023, a Metrology Technician received burn injuries from uncontrolled pressurized steam while removing a pressure transmitter from Water For Injection 831 machine that had no energy sources isolated or locked out.
