29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by a personal fall protection system, such as personal fall arrest, travel restraint, or positioning systems: (a) On or about 11/21/2023 - Shop Area, employees were exposed to a 11 ft., 8 in. fall hazard, while connecting a D ring of the rigging equipment to the hook of an overhead crane when standing on a Conex box.
