29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On or about November 13, 2023, an employee suffered a work-related fatality resulting from a vehicle collision in a work zone which occurred on the eastbound lanes of Interstate 74 near mile marker 133. The employer failed to report the work-related fatality to OSHA within eight (8) hours.
