29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: (a) On or about December 15, 2023, the employer's calendar year 2023 OSHA Form 300 did not include an entry for a November 8, 2023 head laceration that required medical treatment beyond first aid (stitches) after an employee was struck in the head by the bucket of a Ditch Witch (model SK800), mini skid steer.
