29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Worksite, Flexo 8, vacuum roller: On or about November 1, 2023, the employer did not develop procedures to control potentially hazardous energy when a machine operator was repairing a splice on the Print Press Flexo 8: Model number Servo-flex 902/660 5/6+ Star wheel, machine number 18-003-00-000 machine. Employee was exposed to an amputation hazard.
