29 CFR 1926.960(c)(2)(ii)(B): The employer did not ensure that the employee does not remove the rubber insulating gloves and sleeves until he or she is in a position where he or she cannot reach into the minimum approach distance, established by the employer under paragraph (c)(1) of this section: On or about October 24, 2023, employees of Texas Power Suppliers, Inc., were exposed to arc flash burn hazards at the work site as the employer failed to ensure the use of wearing rubber insulating gloves while within the minimum approach distance.
