29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycle: (a)Gates Corporation at 700 East Redwood Boulevard, Brandon, SD 57005: On or about at times prior to October 16, 2023, the employer did not ensure that the UTE Select: Model US38MS-6A (Tube Bender) was adequately guarded at the point of operation. An employee sustained an amputation due to being able to access the point of operation. This condition exposed an employee to caught-in and amputation hazards. Gates Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 1661406, citation number 1, item number 1, and was affirmed as a final order on September 29, 2023, with respect to a workplace located at 1001 Petty Drive, Versailles, MO 65084.
