29 CFR 1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) For the shop or yard area, on or about 10/10/2023 and at times prior to, the employer exposed employees to struck-by/crushing hazards, in that, energy control procedures did not clearly and specifically outline the scope of the procedure and what similar types of equipment it addressed; rather, the procedure just referred, in general, to generic "equipment and machinery". b) For the shop or yard area, on or about 10/10/2023 and at times prior to, the employer exposed employees to struck-by/crushing hazards, in that, energy control procedures did not clearly and specifically outline the types, locations and steps to be followed for the utilization of energy isolating devices for the control of hazardous energy on equipment such as, but not limited to, scissor lifts or similar equipment. Hazardous energy existed in the form of electrical energy, gravitational energy and hydraulic energy on scissor lifts or similar equipment serviced. c) For the shop or yard area, on or about 10/10/2023 and at times prior to, the employer exposed employees to struck-by/crushing hazards, in that, energy control procedures did not clearly and specifically outline the steps to take for the lockout/tagout of scissor lifts or similar equipment serviced.
