29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. Viking Paper Corporation, at a worksite located at 5148 Stickney Ave., Toledo, Ohio 43612: On or about September 4, 2023, the employer did not ensure that employees operating the Triple Stack Preheater/Double Back were provided with and utilized adequate personal protective equipment for the hands and arms. Employees threading product around the heated rollers of the Triple Stack Preheater/Double Back, were exposed to burns.
