29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) Under-construction residential multiplex, roof structure; An employee using a personal fall arrest system while working from exterior wall top plates and/or truss members, at approximate heights ranging between 8-feet to 14-feet above lower levels, was tied-off using a self-retracting lifeline that was attached to the back d-ring of a body harness located near waist-level.
