29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about April 3, 2023, during the installation of roofing material at a job site located at 85 Montrose W. Ave, Copley Ohio 44321; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 20.7 feet by utilizing a fall protection system. Flex Star Certified Roofing Installers Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1638706, citation number 1, item number 1 and was affirmed as a final order on (2/2/2023), with respect to a workplace located at 637 N. Jackson St, Lima Ohio 45801. Flex Star Certified Roofing Installers Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1617492, citation number 1, item number 1 and was affirmed as a final order on (2/9/2023), with respect to a workplace located at 2579 S. Arlington Rd, Akron Ohio 44319.
