29 CFR 1910.119(d)(3)(i)(H): Process safety information pertaining to the equipment in the process did not include the safety systems (e.g., interlocks, detection or suppression systems): The employer failed to maintain accurate process safety information for the safety systems and interlocks on the raw flake conveyance systems to the Extractor. The process safety information documented a safety interlock for an air actuated slide gate at the discharge of the Extractor Plug Screw when conveying goes down although this piece of equipment was not interlocked to close when conveying was stopped.
