29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Haas Door Company, at a worksite located at 320 Sycamore Street, Wauseon, Ohio 43567: On or about August 30, 2023, the employer did not ensure energy control applications steps, isolating electrical and mechanical energy on the #3 Style Line were implemented prior to an employee entering the line's perimeter guarding. Employees performing service and maintenance tasks, such as adjustments to the "flipper arm" while the line was in an energized condition, were exposed to caught in hazards.
