29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 31, 2023, during the installation of roofing material at a job site located at 2177 Old Mill Rd, Madison Ohio 44057; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 12.2 feet by utilizing a fall protection system. RACHELL ROOFING AND CONSTRUCTION LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1614823, citation number 02, item number 01 and was affirmed as a final order on (12/1/2022), with respect to a workplace located at 588 N. Settlers LN, Painesville, Ohio 44077. RACHELL ROOFING AND CONSTRUCTION LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1527820, citation number 01, item number 01 and was affirmed as a final order on (7/23/2021), with respect to a workplace located at 6672 Parma Park Blvd, Parma Heights, Ohio 44130.
