29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about August 30, 2023, employees constructing a covered parking area were exposed to head injuries from falling materials and tools while not wearing protective helmets at 5480 Grant Avenue, Suite 2, Cleveland, Ohio 44125; the employer did not ensure employees working in areas where there was a danger of head injuries from impact, or falling/flying objects were protected by protective helmets.
