29 CFR 1910.28(b)(3)(i)(3) Holes. The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.18(b)(3)(i) (A)-(D): On or about August 25, 2023 the employer failed to ensure that employees in the shop where oil changes are performed were protected from falling a distance of 5 foot 8 inches into a pit. There are 2 holes in the flooring that created a fall hazard into a concrete pit that had metal grates, tools, buckets, and parts used to change oil on vehicles.
