29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 08/28/2023, in the Pit Area; employees were exposed to amputation hazards when Lockout/Tagout (LOTO) procedures were not utilized to perform servicing and/or maintenance activities such as but not limited to; changing blades on the Hyd Mech Vertical Band Saw. b) On or about 08/28/2023, in the Forming Area; employees were exposed to amputation hazards when Lockout/Tagout (LOTO) procedures were not utilized to perform servicing and/or maintenance activities such as but not limited to; changing dies on the ACCUPRESS press brake.
