Location: 37 Tompkins Point, Newark, NJ 07114. a. The employer did not ensure that authorized employees received training on the recognition of applicable hazardous energy sources. On or about August 9,2023, an employee was exposed to caught in hazards and suffered a right index amputation while attempting to remove stuck products from an AMFEC Seydelman-K556 AC-8 mixing machine which was not locked out. Violation occurred on or about August 25, 2023
