Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to struck-by and crush-by hazards. a. ABC Supply Co., Inc. - Findlay, Ohio: On or about August 15, 2023,employees who accessed the Unarco coil rack system, located in the "old building," were exposed to struck-by and crushing injuries in that the storage rack?s columns were not anchored to the floor with anchor bolts. b. ABC Supply Co., Inc. - Findlay, Ohio: On or about August 17, 2023,employees who accessed the pallet rack system, located in the "new warehouse," were exposed to struck-by and crushing injuries in that the pallet storage rack row connectors were not used and columns where items such as fasteners and aluminum drip edge were stored, were not anchored to the floor with anchor bolts. ABC Supply Co., Inc. was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1383150, citation number 1, item number 1 and was affirmed as a final order on August 30, 2019, with respect to a workplace located at 5995 S. Florida Ave., Lakeland, FL 33813. ABC Supply Co., Inc. was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1564021, citation number 1, item number 1 and was affirmed as a final order on April 7, 2022, with respect to a workplace located at 4815 N. Cliff Ave., Sioux Falls, SD 57104. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
