29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 31, 2023, during a roof replacement of a residential home located at 1373 Thoreau Rd, Lakewood, OH 44107, the employer did not ensure employees working on the roof were protected from a fall of approximately 19 feet by utilizing a personal fall arrest system or any other form of fall protection system. Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1656531, citation number 01, item number 001 and was affirmed as a final order on 04/24/2023, with respect to a workplace located at 3748 East Royalton Rd, Brecksville, OH 44141. Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1394075, citation number 02, item number 001 and was affirmed as a final order on 06/07/2019, with respect to a workplace located at 3710 Longwood Ave, Parma, OH 44134. Lovell Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1263203, citation number 01, item number 001 and was affirmed as a final order on 10/17/2017, with respect to a workplace located at 3174 W 54th Street, Cleveland, OH 44102.
