29 CFR 1910.178(l)(3):Powered industrial truck operators did not receive initial training regarding operating instructions, warnings, and precautions for the types of truck the operator was authorized to operate: Location: Warehouse On or about July 20, 2023, employees operating a Yale Model GLC0030CENUAE077 forklift and a HELI DC20 Model 3BA20F electric powered pallet jack were not trained to operate them exposing employees to injury and death.
