29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 19, 2023, subcontractor employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 2705 Northview Road, Rocky River, Ohio 44116. The subcontractors' employees were using an extension ladder to access the roof and the controlling employer did not ensure that the ladder extended three feet above the landing area.
