29 CFR 1910.28(b)(3)(iv): Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toe boards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used; At an establishment located at 121 Cedar Street, in Tigerton, Wisconsin; (a) On or about July 19, 2023, located outside the Sawmill, the two fixed ladders on either side of the yellow platform used by employees to inspect the level of material in the trailers did not have a self-closing gate or offset exposing employees to fall hazards over six feet. (b) On or about October 13, 2023, located outside the Planer Building, the two fixed ladders on either side of the yellow platform used by employees to inspect the level of material in the trailer(s) did not have a self-closing gate or offset exposing employees to fall hazards over six feet. (c) On or about July 28, 2023, in the Chipper Building, the black fixed ladder used to access an elevated platform for the dust blower did not have a self-closing gate or offset exposing employees to fall hazards of approximately 79.5 inches.
