29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: Facility: An evaluation was not conducted as required. A Supervisor was operating a stand-up forklift truck without current evaluation. The U. S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number1455314, citation number 01, item number 01 and was affirmed as a final order on 03/03/2020, with respect to a workplace located at 1150 N.
