29 CFR 1910.140(c)(13)(ii): Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system. Location: 10 Downey Road Sharon, CT 06069. On or about July 6, 2023, the employer did not ensure that a qualified person was present during the design, installation, and use of the anchorage system being used. Workers onsite that were responsible for designing, installing and using the anchorage system for the SRT climbing method that was in use, did not possess a recognized degree, certificate, or professional standing nor did they have extensive knowledge, training, and experience, and had not demonstrated the ability to solve or resolve problems related to the fall system they were using
