29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph: a) Worksite - On or about June 29, 2023, employees used an approximately 12 foot tall extension ladder (Werner) on top a of a mobile baker scaffold (Metaltech, Model: I-CISC) for painting without ensuring the ladder and scaffold were prevented from falling and tipping over exposing the employees to fall hazards.
