29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. On or about June 15, 2023, at the location of SA Recycling 3101 Varley Ave. SW, Canton, Ohio 44706: An employee of SA Recycling LLC. was exposed to a fall hazard of approximately 24 feet while cleaning debris from the Shredder Building rooftop and incorrectly utilizing the personal fall arrest system due to lack of appropriate training.
