1910.179(j)(1)(ii)(a) Frequent inspection - Daily to monthly intervals. OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to serious crushing type injuries associated with operation of the underhung crane/hoist that had a damaged safety latch on the hook. a) On or about June 26, 2023, the employer did not ensure that the hook, located on the Harrington 3 ton hoist in the Melt department, had a hook latch that was not being inspected frequently, in that there was not a functional safety latch mechanism in place on the hook to close the hook's throat and secure loads when the melt employees operated the hoist to move materials. Employees were exposed to crushing injuries associated with being struck-by a fallen load while operating the hoist with the damaged safety latch.
