OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards: a) On or about June 23, 2023, employees operated a Clark C45L forklift, serial number P455L-0231-12011 VNF without a seatbelt or restraint. b) On or about June 23, 2023, employees operated a Hyster 100 Fortis forklift marked SP-1forklift without a seatbelt or restraint. c) On or about July 12, 2023, employees operated a Toyota forklift without a seatbelt or restraint.
