29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about June 18, 2023 employees were exposed to tripping and chemicals hazards while working in the chemical pavilion due to an unguarded 24 x 24 x 12 inch retainment hole.
