29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident a) Facility: On or about June 14, 2023, employees were exposed to struck-by hazards, in that an employee incurred fatal injuries from being struck by a vehicle at 9:35 p.m. The employer reported the fatality to OSHA on June 15, 2023 at 9:56 p.m., 24 hours, 21 minutes after the fatality.
