29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 20, 2023, during the installation of roofing material at a job site located at 850 17TH ST SW, Massillon, Ohio 44647; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 12.8 feet.
