Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) On or about June 15, 2023, employees were required to store, add, and pick heavy items such as washing machines, clothes dryers, and microwave ovens from storage racks that five levels high. The storage rack system is made up of steel structural members in the form of a frame, post, and beams to form a bay. Two separate vertical posts on different rows had been struck by equipment and were damaged and bent near the work floor. This condition exposed employees to struck-by hazards. Feasible and acceptable abatement methods include, but are not limited to: 1) Conduct regular visual inspections of the racking system to identify damaged and/or bent structural framing and fix as needed. 2) Install physical barriers near row ends to prevent forklifts and other mechanized equipment from damaging vertical rack posts.
