29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: At the facility located at 3019 Scanlon Avenue, Salina, Kansas, the employer failed to properly name the Accumulator vessel in room 901 on the CP-2 engine room monitor. The Accumulator was named on the computer monitor as +10 F Recirculator. The process hazard analysis did not address mislabeling of ammonia vessels that can lead to human errors during the operation of the ammonia refrigeration system. Employees engaged in operating, inspecting, testing, and maintaining activities in the ammonia refrigeration system at the facility were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury.
