29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: The employer is failing to protect employees from struck-by hazards associated with the operation of powered industrial vehicles. This violation was most recently documented on or about June 14, 2023 at the U.S. Postal Service located at 615 6th Ave SE, Cedar Rapids, Iowa. The employer did not ensure that an operator's performances were being evaluated at least once every three years when operating a forklift. U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1589962, citation number 1, item number 1a and was affirmed as a final order on September 7, 2022 with respect to a workplace located at 910 1st St S, Hopkins, Minnesota 55343.
