29 CFR 1926.651(k)(1):An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift. Inspections were not made after every rainstorm or other hazard-increasing occurrence. Trench: On or about June 13, 2023, and at times prior thereto, an employee was exposed to struck-by hazards when working in an excavation that was not inspected to ensure that the system was pressurized prior to the start of the shift.
