29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a): Facility - Dough Divider - The employer failed to develop, document, and utilize procedures for the control of hazardous mechanical energy. The employer failed to ensure that employees were locking out energy sources before placing hands inside the dough divider. An employee sustained two partial finger amputations due to contact with the automatic blade when reaching inside to remove excess dough, on or about June 5, 2023. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
