29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: Location: Asphalt Manufacturing Plant: On or about June 12, 2023, in the Roll Feed Area the unguarded spindle on the unwind stand posed a nip point hazard that was not guarded exposing employees to lacerations and amputations. On or about June 12, 2023, in the Roll Feed Area the rollers were not guarded, exposing employees to lacerations, crush injuries, and amputations. On or about June 12, 2023, the roller end of the Accumulator's spindle was not guarded and posed nip point hazard, exposing employees to lacerations and amputations. On or about June 12, 2023, the roller end of the Saturator's spindle was not guarded and posed a nip point hazard, exposing employees to lacerations and amputations. On or about June 12, 2023, the Cooler Section's Press Roller was not guarded and posed a caught-in hazard, exposing employees to lacerations, crush injuries, and amputations. On or about June 12, 2023, the roller start of Coater's spindle was not guarded and posed a nip point hazard, exposing employees to lacerations and amputations. On or about June 12, 2023, the Paint Section's Dancer Roller was not guarded and posed a caught-in hazard, exposing employees to lacerations, crush injuries, and amputations.
