29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 06/07/2023 at a paving project located on Interstate 90, westbound, near mile marker 400, Pembroke, New York; employees performing milling and paving operations on the New York State Thruway were working around dump trucks that were backing up to milling machines and paving machines. A dump truck backing up to a paving machine struck an employee standing behind the truck. Employees were not instructed to recognize or avoid blind spot hazards around dump trucks and construction equipment. ABATEMENT DOCUMENTATION REQUIRED
