1926.502(i)(3) All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. A) 90 E. Scott Ave, Building 80B 2nd Floor, Rahway, NJ 07065 - A cover used to protect a hole on the second floor was not secured to prevent displacement and exposed employees to fall hazards. An employee fell through the hole approx. 18 ft. and suffered serious injuries. Incident occurred on June 7, 2023.
