29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative. a) Innovative Metals Inc.- Throughout the manufacturing area: The employer did not establish a Lock Out/Tag Out Program consisting of energy control procedures, employees training, and periodic inspections to protect employees against the unexpected energization of equipment when servicing, making adjustments, replacing parts, tooling/die changes, cleaning or unjamming machines. Machinery includes, but is not limited to, equipment on the Green Rollformer Line, including the Bosh Sheer and Zib-Rib and the Lockformer Rollforming machine, occurred on or about 06/07/2023
