29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations such as opening process equipment. On or about May 23, 2023, at the Prime G Reboiler Pass 4 Control Valve at the Wynnewood Refinery in Wynnewood, Oklahoma, employees were exposed to fire and chemical release hazards when safe work practices that fully defined the scope and limitation of the work to be performed were not effectively implemented by the maintenance department.
