29 CFR 1910.23(c)(4): The employer did not ensure ladders are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement. On or about May 3, 2023, an employee was exposed to fall hazards while working from a multi-position ladder placed on landscaping rocks and the employer did not ensure the ladder was placed on a stable surface.
