29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a.) Bashlin Industries, Inc. (Perking punch press; Model #2-A; Serial #19949) - On or about 4/25/23, and at times prior thereto, the the two-hand control guard feature did not function properly and an employee was injured. b.) Bashlin Industries, Inc (Perking punch press; Model #2-A; Serial #19949) - On or about 4/25/23, and at times prior thereto, the two-hand control was missing the ring /collar guard around each two hand control/trip to prevent bypassing one of both of the buttons. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
